Quick Answer
The Attendance Allowance claim form is the AA1 — a 32-page paper form. You can download it from our resources page. Post a completed form to Freepost DWP Attendance Allowance (no stamp needed). Around 60% of self-completed forms are rejected because they describe a diagnosis, not daily care needs. Our HCPC-registered Occupational Therapists complete the form for you from a clinical assessment. Call 0208 081 4500 or start at attendance-allowance.co.uk. There is no fee unless your claim succeeds.
What Is the Attendance Allowance Form?
The official Attendance Allowance form is the AA1 claim form. It is the document the DWP uses in England and Wales to decide whether you qualify and at which rate. The form asks about your health conditions — and, more importantly, the help you need during the day and at night.
“Attendance allowance form”, “attendance allowance claim form”, and “attendance allowance form pdf” all refer to the same AA1. Having the PDF is useful. Completing it in the way the DWP scores it is what actually wins the award.
The same form is used when a fixed-term award is renewed, see Attendance Allowance renewal. How to Get the Attendance Allowance Form (PDF)
The simplest way to get a copy is from our site. We keep the official forms together so you are not hunting across GOV.UK:
Download the Attendance Allowance form
Get the AA1 (and related guides) from our resources page. Print it, keep a spare copy, and do not post anything until you are sure the answers match your real care needs.
If a family member is helping you, they can still download the form — but the declaration must be signed by you or your appointee. See who can fill in the Attendance Allowance form.
Prefer not to tackle 32 pages yourself? You do not have to. Call 0208 081 4500 and our team will explain how an Occupational Therapist completes the AA1 for you.
Where to Send the Completed Form
If you are posting the form yourself, use the DWP Freepost address. No stamp is needed.
Stuck on a question before you send it? See how our Occupational Therapists can help with the formNo stamp. No postcode.
If a post office asks for a full address: Attendance Allowance Service, Mail Handling Site A, Wolverhampton, WV98 2AD. Photocopy or photograph every page before you post it.
When you use Attendance Allowance Services, you do not have to manage this alone. After the OT assessment, we complete the form, you review and sign it, and we guide you through submission so nothing is missed. Call 0208 081 4500 if you would rather we handle it.
Why Most Attendance Allowance Forms Are Rejected
The form looks like a medical questionnaire. It is not. The DWP scores frequency, duration, and consequence of help — not the name of your condition. Writing “I have arthritis” is treated as if you have no care needs.
That is why people who genuinely qualify are refused. Being on the list of 57 conditions is not enough. The #1 barrier is how the form is written — see what actually stops you from getting Attendance Allowance.
| What the form asks | What actually gets an award |
|---|---|
| Your illnesses and medication | What you cannot do safely without help, how many times a day, how long it takes |
| “Do you need help washing / dressing / eating?” | Specific examples on a typical bad day, plus what would happen if nobody helped |
| Night-time needs | How often you wake, how long help lasts, who helps you (this is what unlocks the higher rate) |
GP letters help, but they do not replace the form. See what evidence to include.
What Our Occupational Therapists Do With the Form
Attendance Allowance Services is built around HCPC-registered Occupational Therapists — not unregistered form-fillers. An OT is trained to assess how a condition affects mobility, transfers, washing, dressing, eating, toileting, medication, safety, and night-time care. That is exactly the language the DWP uses to score the AA1.
When you work with us, our OTs:
- Carry out a clinical assessment of your (or your relative’s) daily care needs
- Capture needs people often leave off the form — especially supervision and night-time help
- Write the AA1 using frequency, duration, and consequence — the detail decision-makers look for
- Complete the full 32-page form for you to review and sign
- Support you through to submission so the form is not posted incomplete
Our success rate is 99%. There is no fee unless you are awarded the benefit.

How Much Attendance Allowance Is If the Form Succeeds (2026/27)
Lower rate £76.70 a week (day or night needs). Higher rate £114.60 a week (day and night). That is up to £5,959 a year, tax-free, and it does not reduce Pension Credit. Full figures: Attendance Allowance rates 2026. In fact it can increase any existing Pension Credit you may already receive to the full amount.
Delaying the form costs money you usually cannot backdate. If you are unsure, a two-minute check or a call to 0208 081 4500 is faster than posting a weak AA1 and waiting months for a refusal.
Frequently Asked Questions
Where can I download the Attendance Allowance form PDF?
From our resources page. Print it and keep a copy. The PDF is the same AA1 as the posted pack. If you would rather not complete it yourself, call 0208 081 4500 and an Occupational Therapist can do it from an assessment.
How long does the Attendance Allowance form take to complete?
Several hours if you do it properly, often spread over days. Rushed answers are a common reason for refusal. Our OTs gather the detail in a structured assessment so the form is complete and consistent.
Can Attendance Allowance Services fill in the form for me?
Yes. That is the core of the service. HCPC-registered Occupational Therapists assess your care needs and complete the AA1. You review and sign it. There is no fee unless the application is successful. Call 0208 081 4500 or start at attendance-allowance.co.uk.
What if my Attendance Allowance form is rejected?
You can ask for a mandatory reconsideration, then appeal. Many refusals are form-quality issues, not true ineligibility. We specialise in putting the clinical detail back in. Guide: what to do if your Attendance Allowance is rejected — or call us on 0208 081 4500.
